Job summary

Valor Hospice is seeking an Admission RN in Fort Worth, TX to perform admissions and provide nursing care at patients' residences. The role requires coordinating with the Interdisciplinary Team, families, and referring agencies, with flexible scheduling and on-call duties.

Monday–Friday, 9am–6pm, some variation by admissions and supervisor assignments. The RN must have an RN license, 2+ years clinical RN experience, and 1 year hospice experience, along with reliable transportation and compliance

Qualifications

• Education: Diploma/Associate/Baccalaureate in Nursing.
• Licensure: Current RN license and valid driver's license.
• Experience: 2+ years as RN in clinical care; 1 year hospice experience.
• Skills: Nursing skills, regulatory knowledge, good interpersonal ability.

Responsibilities

• Perform admissions and administer nursing care for patients at home.
• Coordinate care with Interdisciplinary Group/Team, patient, family, and referring agency.
• Maintain office hours, participate in on-call scheduling, and meet on-call duties.
• Communicate with physicians involved in patient care and submit required paperwork timely.
• Demonstrate ongoing professional growth and participate in QAPI programs.
