Job summary

Valor Hospice is seeking a Registered Nurse to perform admissions and provide nursing care in patients' homes in the Houston area. You will coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agencies while managing documentation and on-call duties.

Qualified candidates have an RN license, 2 years of clinical RN experience, and at least 1 year in hospice. Strong interpersonal skills and reliable transportation are essential to excel in this role.

Qualifications

  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current State license as a Registered Nurse; Current driver's license.
  • Experience: Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
  • Skills: Nursing skills per standard practice; knowledge of state and federal regulations.
  • Good interpersonal skills and reliable transportation.

Responsibilities

  • Assess hospice patients and identify physical, psycho-social, and environmental needs.
  • Develop and document the plan of care respecting assessments.
  • Provide nursing care with infection control measures.
  • Collaborate with the Interdisciplinary Team to meet patient/family needs.
  • Communicate with attending physicians and hospice physicians.
  • Submit required paperwork timely and maintain competency; participate in QAPI.

Primary functions are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume the responsibility for coordination of care.

Essential Functions
  • Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psycho social, and environmental needs.
  • Develop and document the plan of care in correlation with Assessment
  • Provides nursing care utilizing infection control measures (OSHA)
  • Work with Interdisciplinary Group/Team to ensure patient/family needs are met.
  • Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care.
  • Timely submission of all required paperwork.
  • Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice's QAPI program.
  • Promotes the Hospice's philosophy and administrative policies.
  • Meets productivity standards.
  • Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
  • Provides effective communication to patients, their family members, team members, and other health care professionals.
