Job summary

Heart to Heart Hospice in Fort Worth, TX seeks an Admissions RN to deliver compassionate hospice care. You will conduct initial patient/family assessments, develop care plans, and collaborate with the care team to determine eligibility and appropriate services.

Ideal candidates have a nursing degree (RN), possess an active RN license, a valid driver’s license, and 1–3 years in clinical care or hospice/home health. Full benefits are offered, including a company car for qualifying individuals.

Qualifications

• Must have Nursing Diploma/ADN/BSN.
• Current State RN license.
• Valid driver's license and auto liability insurance.
• 1–3 years RN experience in clinical care, hospice or home health preferred.

Responsibilities

• Review clinical referral information and respond to requests and inquiries as appropriate.
• Collaborate with Patient Care Manager and physicians regarding eligibility for hospice services.
• Conduct initial meeting with patient and family about hospice care and obtain consents.
• Provide initial comprehensive assessment of physical, psychological, social and spiritual needs.
• Develop initial care plan with goals and interventions based on nursing assessment.
• Maintain regular communication with Intake Coordinator, Patient Care Managers and core team.
• Timely submission of all documentation and records.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• One to three years of experience as an RN in a clinical care setting, hospice or home health preferred.

What You Will Do:

• Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Review clinical referral information and responds to requests and inquiries as appropriate
• Collaborate with Patient Care Manager, attending physician and hospice physician regarding the eligibility and appropriateness of client for hospice services
• Conduct initial meeting with the patient and family over hospice service and philosophy and obtains consents for care
• Provide the initial comprehensive assessment of immediate physical, psychological, social and spiritual needs of the hospice patient and family and initiates appropriate interventions
• Develop an initial care plan that establishes immediate goals and interventions, based on nursing assessment and collaborates with the attending HMD and core team
• Maintain regular communication with the Intake Coordinator, Patient Care Managers, RN Case Manager and core team to review patients and help with managing needs (this may include unusual or potentially problematic patient/family issues)
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
