Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service field.
• Knowledge of available community resources is preferred.
• Previous work with the SMI population is highly preferred.
• Must have valid driver's license and meet auto insurance requirements.

Responsibilities

• Provide community-based case management and skills training services.
• Evaluate individuals to determine psychiatric stability and daily living skills.
• Develop Person-Centered Treatment Plans based on assessment results.
• Participate in weekend/holiday coverage rotation after the provisional period.
