Job summary

Southwestern Behavioral Healthcare in Evansville, IN is seeking a full-time Case Manager to provide Assertive Community Treatment services through the Community Support Services team. You will assess, plan, and deliver recovery-oriented, community-based care for adults with complex needs.

The role emphasizes collaboration, strong communication, and adherence to program guidelines. A bachelor’s degree in a human services field is required, with background checks and a valid driver’s license

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or related Human Service field.
• Knowledge of available community resources preferred.
• Experience with SMI population highly preferred.
• Excellent communication, writing, time management and organizational skills.
• Valid driver’s license and auto insurance meeting Indiana minimum requirements.
• Background checks including county/state checks, sex offender registry, and drug screen required.

Responsibilities

• Provide face-to-face community-based case management and skills training from a recovery/person-centered perspective.
• Evaluate psychiatric stability and daily living skills using the ANSA instrument.
• Provide community-based skills training and development for adult clients.
• Coordinate assessments and translate needs into a Person-Centered Treatment Plan.
• Participate in CSS weekend/holiday coverage after provisional period.
