Qualifications

• Active and unrestricted medical license in the applicable state.
• Experience providing addiction medicine services for complex populations.
• Ability to develop relationships with physicians and providers.
• DEA licensure and prescriptive authority in state(s) of practice.
• On-site work with diverse care teams and strong communication.

Responsibilities

• Provide initial evaluation, diagnosis, treatment and ongoing support for individuals with addiction and related health conditions.
• Complete documentation to support professional billing for services.
• Educate patients on illness, prognosis and care plans.
• Interpret test results and provide feedback to patients.
• Educate and counsel patients regarding medication compliance.
• Collaborate with an interdisciplinary team to provide medical and behavioral health services.
• Conduct patient follow-up visits to monitor progress with medication or treatment.
• Coordinate with PCPs and specialists as needed.
• Evaluate lab results and discuss abnormal findings with APPs.
• Review progress and adjust treatment plans as necessary.
• Maintain licensure, certifications and CME credits.
