Job summary

Soleo Health is seeking a Full-time Therapy Care Pharmacist to support our Acute Therapy Care Management Team in Hoover, AL. The role focuses on coordinating care for patients with specialty therapies, collecting data, and collaborating with prescribers to optimize treatment plans.

Responsibilities include dispensing medications, supervising therapy care staff, and ensuring compliance with state and federal regulations. On-site schedule is Monday–Friday, 8:30am–5:30pm with on-call rotation.

Qualifications

• Must have PharmD or R.Ph with clinical experience.
• Postgraduate residency or training preferred.
• Active state licensure required.
• CPR+ certification required.
• Prior home infusion or specialty pharmacy experience preferred.

Responsibilities

• Collect clinical data for patients, caregivers, and nurses to monitor therapy efficacy and safety.
• Collaborate with prescribers to implement or modify treatment plans.
• Act as a clinical resource to patients, prescribers, and team members.
• Coordinate activities with pharmacy, nursing, reimbursement, and sales teams.
• Document all patient interactions and care coordination in the system of record.
• Dispense specialty medications and supplies.
• Supervise therapy care staff and ensure compliance with laws and policies.
