Soleo Health is seeking a Full-time Therapy Care Pharmacist to support our Specialty Therapy Care Management Team in Cincinnati, OH. The role focuses on collecting data, implementing care plans, and coordinating with prescribers to optimize patient outcomes.

Requirements include PharmD or R.Ph with clinical experience, residency training, and active licensure. The position emphasizes collaboration, documentation, and compliance with regulations while avoiding weekends or holidays.

Formación

• Experience in home infusion or specialty pharmacy settings.
• Doctor of Pharmacy (PharmD) or equivalent licensure.
• Post-graduate residency or specialty training preferred.
• Active state licensure and CSP preferred but not required.

Responsabilidades

• Collect and assess clinical data to monitor efficacy and safety of specialty therapies.
• Collaborate with prescribers to optimize treatment plans.
• Provide clinical guidance to patients, prescribers, and care teams.
• Coordinate with pharmacy teammates to ensure efficient patient care.
• Document interactions and care coordination in the system of record.
• Dispense specialty medications and supplies; ensure compliance with laws.
