Fairview Health Services is seeking an Acute Social Worker (MSW) to join Southdale Hospital in Edina, MN. The role is 0.7 FTE (56 hours biweekly) with a schedule from 7:30 a.m. to 4:00 p.m.
and one weekend per month. The social worker will work on-site with an interdisciplinary team to assess, plan, and discharge patients from the hospital. The position requires a Master of Social Work and Minnesota licensure eligibility (LGSW or LISW upon hire).
Fairview is looking to hire an Acute Social Worker (MSW) to join our team at Southdale Hospital in Edina, MN. This is a benefit-eligible, 0.7 FTE position (56 hours every two weeks). The anticipated schedule is 7:30 a.m. to 4:00 p.m., and candidates must be available to work one weekend per month.
The acute social worker works on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute social worker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The social worker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.
