Job summary

Fairview Health Services in Minnesota seeks a licensed behavioral health clinician to perform diagnostic assessments, develop treatment plans, and coordinate multi-disciplinary care for patients in Maplewood and Edina. The role includes therapy with individuals, couples and families, and discharge planning.

You will work under supervision, participate in interdisciplinary teams, and ensure documentation meets regulatory requirements.

Qualifications

  • M.A./M.S. in Psychology, Social Work, or closely related field.
  • Completion of clinical education and internship as required by graduate program.
  • Be working towards licensure by Board as LICSW, LMFT, LPCC or LP.
  • Basic Life Support certification where required for departments.

Responsibilities

  • Demonstrates clinical competence in treatment planning and case management.
  • Collaborates with patient, family and interdisciplinary team to develop treatment plans.
  • Performs interdisciplinary assessments and updates weekly.
  • Coordinates discharge planning and transition processes.
  • Provides individual, group, couples, and family therapy.
  • Maintains thorough, timely documentation meeting regulatory requirements.
  • Engages in evidence-based practice and participates in related activities.

Job Overview

This role focuses on diagnostic assessments and referals to programmatic care. Trainee conducts diagnostic assessments under the supervised practice of fully licensed and system credentialed supervisor, determines clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary teams and all other contacts. Functions as an interdisciplinary team member. Accepts supervision and consultation with a licensed supervisor for the functions of this position in their clinical practice.

Schedule: Mon-Fri, 8-4:30

Location: Maplewood and Edina

Responsibilities
  • Demonstrates clinical competence in the treatment planning process.
  • Facilitates treatment planning and oversee management of each case.
  • Coordinates/facilitates the development of treatment plan by collaborating with patient, family and interdisciplinary team.
  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjust treatment plan as necessary.
  • Demonstrates clinical competence to assess individual patient needs.
  • Completes an interdisciplinary assessment tool with each patient.
  • Updates interdisciplinary assessments tool each week.
  • Demonstrates clinical competence in disposition planning.
  • Interfaces with government agencies such as court, ombudsmen, county services
  • Completes referral process for next level of care
  • Communicates to other providers in the community
  • Sets up intake appointments
  • Completes needed paperwork to facilitate this process i.e. applications, letters of recommendation, etc
  • Demonstrates clinical competency to partner with multi-disciplinary team in the treatment planning process.
  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjusts treatment plan as necessary.
  • Communicates frequently and effectively with interdisciplinary team to ensure continuity and coordination of care.
  • Documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.
  • Demonstrates competence in developing a clinically relevant transition/discharge plan.
  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan
  • Develops discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
  • Documents thoroughly, timely, legible and clinically appropriate.
  • Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups.
  • Provides individual, couples and family therapy.
  • Documents thoroughly, timely, legible and clinically appropriate.
  • Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
  • Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities:
  • Utilizing evidence based policies in clinical practice
  • Attends and participates in learning opportunities about evidence based practice
  • Membership in hospital or unit based practice council/research council
  • Evidence based practice project (e.g. policy development or poster presentation)
  • Journal club, journal article critique and circulation
  • Literature review
  • Development of standard/pathway/guideline for patient care
  • Presentation

Required Qualifications
  • M.A./M.S. in Psychology, Social Work, or closely related field
  • Completion of clinical education and internship as required by graduate program.
  • For Behavioral inpatient units, completion of Behavioral Control Situation training, and retraining every two years or as often as required.
  • For Behavioral outpatient units, De-escalation and Self-Defense training and retraining as required.
  • Basic Life Support Certification prior to end of orientation for Behavioral units.
  • Be working towards licensure by Board as LICSW, LMFT, LPCC or LP. Terminal license required within; 3 years for LICSW, LMFT, LPCC; 5 years for LP of hire.
  • Basic Life Support (American Heart Assoc or Red Cross) Completion of BLS is a requirement before start date for all new hires in the following Departments: Adult SUD IOP- St. Josephs, St. John’s, UMMC, Bethesda, Adolescent Dual Programs- Crystal, Maplewood, Elk River, Adult Lodging plus, Adolescent Residential Program, Adolescent Day Therapy- UMMC, St. John’s, Child Day Therapy and Adult Mental Health Outpatient Programs
