Job summary

Fairview Health Services is seeking an Acute Social Worker (MSW) to join Southdale Hospital in Edina, MN. The role is 0.7 FTE (56 hours biweekly) with a schedule from 7:30 a.m. to 4:00 p.m.

and one weekend per month. The social worker will work on-site with an interdisciplinary team to assess, plan, and discharge patients from the hospital. The position requires a Master of Social Work and Minnesota licensure eligibility (LGSW or LISW upon hire).

Qualifications

• Master of Social Work required.
• Minimum 1 year of healthcare-related experience.
• LGSW or LISW licensure upon hire in Minnesota.
• Preferred: 3 years healthcare experience.

Responsibilities

• Attends daily in-person interdisciplinary care team rounds.
• Conducts thorough psychosocial assessments with patients and/or surrogates.
• Assists care team with surrogate decision maker identification and policy.
• Communicates with interdisciplinary team to ensure care coordination.
• Provides emotional support and counseling for adjustment to illness, bereavement, and end-of-life planning.
• Addresses social determinants of health and connects patients to community resources.
• Facilitates in-person care conferences to support patient/family decision-making.
• Documents interactions thoroughly per department standards.
• Acts as expert on adult and child protection reporting policies.
• Collaborates on individualized discharge planning considering age, culture, diagnoses, and needs.
