Job summary

Dignity Health East Valley Rehabilitation Hospital - Gilbert is seeking an Registered Nurse (RN) to join our acute rehab team in Gilbert, AZ. This Full-Time day shift (3x12, 6a-6p) includes rotating weekends.

Our facility is dedicated to high-quality patient care and collaborative teamwork. The ideal candidate will hold an active RN license in the state, have a nursing degree, and preferably six months of Medical/Surgical experience in an acute care setting.

Qualifications

• Current state RN license.
• Education: BSN/ADN or Nursing Diploma.
• Minimum six months of Medical/Surgical experience in an acute care setting.
• Current state licensure as an RN.
• CPR/BCLS certification required; ACLS & CRRN preferred.

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction.
• Responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
• Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
