Job summary

Dignity Health East Valley Rehabilitation Hospital - Gilbert, in partnership with Lifepoint Health, is seeking a Registered Nurse (RN) for Acute Rehab to provide patient-centered care on a 3x12 schedule.

Join a collaborative team focused on high-quality outcomes, discharge planning, and compassionate care. Minimum six months Medical/Surgical experience preferred; BSN/ADN or Diploma required; CPR/BCLS mandatory; ACLS/CRRN preferred.

Qualifications

• Education: Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
• Experience: Minimum six months of Medical/Surgical experience in an acute care setting preferred.
• License: Current state licensure as a Registered Nurse.
• Certifications: CPR/BCLS certification required; ACLS & CRRN preferred.

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction.
• Responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
• Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
