Job summary

Dignity Health East Valley Rehabilitation Hospital - Gilbert is seeking a Licensed Practical Nurse (LPN/LVN) for acute rehab, nights, full-time. You will provide direct patient care and support the rehab team to help patients progress toward independence.

You must hold an Arizona LPN/LVN license, have CPR/BLS, and ideally IV certification; minimum six months of medical/surgical acute care experience is preferred.

Qualifications

• Current LPN/LVN license in the state of Arizona.
• Graduate of an accredited LPN/LVN program.
• CPR/BLS certification required.
• IV Certification highly preferred or must be obtained within 6 months.
• Minimum six months of Medical/Surgical experience in an acute care setting preferred.

Responsibilities

• Administer medications safely and accurately based on patient condition.
• Assist RN with initial assessments and patient reassessments per policy.
• Identify and implement rehabilitation nursing interventions.
• Perform timely and accurate Quality Improvement assessments under RN supervision.
• Deliver care based on patient response and update the interdisciplinary plan accordingly.
• Develop and implement individualized teaching plans for patients and families.
