Job summary

Brio Living Services is seeking an Acute Home Care Participant Care Assistant, CNA in St. Joseph, MI. The full-time position covers in-home care, day center support, and on-road training for home health staff. Schedule is 8am–4:30pm, Monday–Friday, 40 hours per week.

The role requires CNA certification, driving flexibility, and up-to-date CPR/BLS. Experience with frail or elderly individuals and home health settings is preferred. Excellent patient care and teamwork are essential.

Qualifications

• CNA certification is required.
• One year of experience with frail or elderly individuals.
• Direct care experience in home health, long-term care, acute care, or similar settings.
• Valid Michigan Driver’s License with a clean driving record.
• Current CPR or BLS certification or willingness to obtain.

Responsibilities

• Provide direct care to participants in homes, assisting with ADLs, health monitoring, mobility, and safety checks.
• Follow individualized care plans and communicate observations to nursing staff.
• Support participants to maintain independence at home.
• Assist day-center participants with ADLs, meals, mobility, social engagement, and supervision.
• Provide support to clinical team during peak times.
• Maintain a safe, clean, supportive environment for all participants.
• Deliver hands-on training and coaching to home health team members in the field.
• Demonstrate proper care techniques and safety protocols.
• Document care provided and any participant status changes.
• Report concerns to the care team promptly.

Department: PACE of Southwest Michigan | St. Joseph, MI

What You’ll Do:

The Acute Care PCA provides flexible, high-quality support across multiple care settings, including in-home care, the day center, and training home care team members. This role ensures continuity of care by assisting participants with daily living activities, supporting clinical team members, and delivering hands‑on training to home health team members while on the road.

Key Responsibilities

• Provide direct care to participants in their homes, including assistance with activities of daily living (ADLs), basic health monitoring, mobility support, and safety checks.
• Follow individualized care plans and communicate observations to nursing or clinical staff.
• Support participants in maintaining independence and well‑being within their home environment.
• Assist participants attending the day center with ADLs, meal assistance, mobility support, social engagement, and general supervision.
• Provide additional support to day center clinical team members during peak times.
• Maintain a safe, clean, and supportive environment for all participants.
• Assist in delivering hands‑on training, coaching, and support to home health team members in the field.
• Demonstrate proper care techniques, safety protocols, and participant support strategies.
• Document care provided, observations, and participant status changes.
• Report concerns related to participant health, safety, or functional status to the care team promptly.

Skills, Knowledge and Expertise:

• CNA certification, required.
