Job summary

UCI Health seeks a Clinical Nurse to collaborate with interdisciplinary teams to develop and implement care plans for an assigned patient group. The role includes assessing, planning, intervening, and advocating for patients across the continuum of care.

Responsibilities include evaluating outcomes and discharge readiness, mentoring students and new nurses, and serving as a Patient Care Coordinator as needed. A BSN is required, with at least 2 years of acute care nursing and telemetry experience.

Qualifications

• Minimum 2 years acute care nursing experience with telemetry experience or completion of UC Irvine Medical Center Clinical Nurse I SSDU Program.
• Ability to work variable shifts including evenings, nights, weekends, and holidays.
• Ability to read, write and communicate effectively in English.
• Preferred: bilingual skills to communicate with patients and families.

Responsibilities

• Collaborate with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients.
• Provide professional nursing care, assess and plan, intervene, and advocate for patients throughout the continuum of care.
• Evaluate patient outcomes, effectiveness of the care plan, and readiness for discharge; revise plans or make referrals as needed.
• Serve as a role model and mentor to students and new nurses; function as a Patient Care Coordinator as requested.
