Advocate Trinity Hospital is seeking a Part-time Speech Language Pathologist (PRN) to support acute care rehabilitation services across Advocate Trinity and South Suburban Hospitals. The role requires providing evaluations, plans of care, and treatment to patients with speech, language, voice, cognitive and swallowing disorders.

Master's degree in Speech Pathology and a state license are required, with BLS certification.

Qualificações

• Master's degree in Speech Pathology from an accredited school.
• Speech Language Pathologist license in the state of practice.
• Basic Life Support (BLS) for Healthcare Providers.

Responsabilidades

• Interprets physician referrals and conducts speech and language evaluations for patients with speech, language, voice, cognitive and swallowing disorders.
• Establishes a written plan of care and implements treatment based on evaluation results and goals, monitors outcomes and adjusts plans as needed.
• Performs patient discharge planning, including home programs, family teaching, and referrals to community services; communicates with stakeholders to ensure optimal care.
