Job summary

Advocate Trinity Hospital in Chicago is seeking a per diem Speech-Language Pathologist for an acute care rehabilitation setting between Advocate Trinity and South Suburban Hospitals. You will evaluate patients, develop and implement treatment plans, and coordinate with physicians and care teams.

Master’s degree in Speech Pathology and Illinois SLP licensure (or eligibility) with CCC-SLP certification are required. BLS is also required or as determined by leadership.

Qualifications

• Master’s degree in Speech Pathology from an accredited program.
• Current CCC-SLP certification or eligibility.
• State licensed Speech Language Pathologist in Illinois or eligible prior to hire.

Responsibilities

• Interprets physician referrals and conducts a speech and language evaluation of patients with various speech, language, voice, cognitive and swallowing disorders.
• Establishes a written plan of care and implements individualized treatment based on evaluation results and goals.
• Completes required documentation, discharge planning, and communication with physicians and care teams.
• Maintains productivity and adheres to billing and scheduling policies.
