Job summary

ChristianaCare in Delaware is seeking a Social Worker to join the Care Management Triad Team. The role focuses on care coordination, discharge planning, and resolving psychosocial barriers to support safe patient transitions.

The position requires an MSW/MSS and licensed CSW or MSW as applicable by Delaware rules, with acute care experience preferred. A sign-on bonus up to $15,000 may apply based on the two-year commitment.

Qualifications

• Master’s degree in Social Work (MSW) or Master of Social Services (MSS) required.
• Licensed CSW or MSW as required by Delaware prior to hire date.
• Acute Care Social Work experience preferred.

Responsibilities

• Collaborate with RN Case Manager to drive discharge planning.
• Resolve psycho-social barriers and support discharge needs.
• Participate in care coordination within the triad team model.
