Qualifications

• Provides and directs safe, effective nursing care across acute medical-surgical units.
• Monitors and responds to patient data using telemetry where applicable.
• Collaborates with physicians and health care team to plan and implement care.

Responsibilities

• Performs initial assessment within 30 minutes of admission.
• Develops and updates outcome-based care plans.
• Coordinates care with case managers, social workers and other staff.
• Ensures discharge planning and patient/family education.
