Job summary

Wellstar Health System is seeking an experienced RN Complex Care Coordinator for the Cobb Hospital Care Coordination team in Georgia. This role focuses on assessing and coordinating complex patient transitional care across the continuum, with strong collaboration among patients, families, physicians and care teams.

Responsibilities include developing discharge plans, addressing psychosocial and financial needs, and mentoring fellow coordinators.

Qualifications

• Active Registered Nurse license in Georgia.
• Minimum 3 years of healthcare experience in acute care, skilled care or related settings.
• Minimum 2 years in care coordination within acute care setting.

Responsibilities

• Assess complex patient transitional care needs and coordinate across the continuum.
• Engage with patients and families to ensure discharge plans meet needs.
• Provide expert guidance on psychosocial and resource needs for complex cases.
• Mentor other care coordinators and participate in precepting as needed.
• Collaborate with interdisciplinary team to optimize discharge timing and level of care.
