Job summary

Wellstar Cobb Hospital is seeking an RN Complex Care Coordinator to assess complex patient transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure discharge plans are met.

You will be a resource to the care team, lead discharge planning, participate in interdisciplinary rounds, and mentor new coordinators while honoring patient wishes and social determinants of health.

Qualifications

• Minimum 3 years of experience in healthcare in the acute care setting or related field.
• Minimum 2 years in care coordination in the acute care setting.
• Experience providing comprehensive clinical and psychosocial assessments.

Responsibilities

• Provide comprehensive psychosocial and clinical assessments for complex patients.
• Coordinate discharge planning and post-acute referrals.
• Participate in interdisciplinary rounds to optimize discharge timing and care transitions.
• Precept and mentor new care coordinators and staff.
• Advocate for patients and families, aligning care with their wishes and needs.
