Qualifications

• Minimum 3 years of healthcare experience in acute care, skilled care or community in care coordination.
• Minimum 2 years in care coordination in the acute care setting.
• Relocation assistance for eligible candidates.

Responsibilities

• Assess complex patient transitional care needs and coordinate across the continuum.
• Serve as expert resource on complex psychosocial and discharge needs.
• Coordinate timely, safe discharge plans with patient, physician and care team.
• Participate in Interdisciplinary Rounds to confirm discharge date and transitions.
• Precept and mentor new care coordinators as needed.
• Coordinate with community services and post-acute referrals.
