Qualifications

• Minimum of 3 years in acute‑care healthcare settings.
• Minimum of 2 years in care coordination within an acute‑care environment.
• Strong written and verbal communication with professionalism.
• Self‑directed, can handle stress and fast‑paced environments.
• Knowledge of regulations and community resources.
• Ability to collaborate with physicians, care teams and patients/families.

Responsibilities

• Assess high‑risk patients clinically and psychosocially.
• Develop and implement timely discharge plans.
• Coordinate post‑acute referrals and community resources.
• Advocate for patient autonomy respecting beliefs and culture.
• Participate in rounds to confirm discharge dates and transitions.
• Identify barriers and work with resources to address them.
• Precept and mentor new staff on complex cases.
• Support protocol and performance‑improvement activities.
• Document psychosocial assessments and arrangements in EHR.
