Qualifications

Responsibilities

• Perform initial patient assessment within 30 minutes of arrival.
• Develop and implement outcome-based care plans.
• Coordinate care with case manager, social worker and other disciplines.
• Document discharge instructions and plan for post-discharge needs.
• Manage telemetry monitoring when applicable.
• Ensure patient safety and adherence to policies and procedures.
