Qualifications

Responsibilities

• Performs initial therapy evaluations and re-evaluations to assess patient needs and plan of care.
• Provides therapeutic treatment and monitors patient progress, adjusting plan of care as needed.
• Maintains timely, accurate documentation and communicates results to the care team.
• Collaborates with the interdisciplinary team to coordinate care and discharge planning.
