Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of practice.
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to hospital policies and fosters a cooperative work environment through communication and teamwork.
• Provides patient care assessment and treatment with comprehensive evaluations and realistic goals.
• Documents evaluations, progress notes, discharge notes, and educational activities accurately.
• Maintains legible documentation and contributes to department financial and productivity standards.
• Participates in process improvement to enhance quality and efficiency of services and shares clinical knowledge with staff.
• Communicates effectively with patients, physicians, staff and families, ensuring understanding and timely information.
• Delivers age-appropriate care and maintains competence across patient populations served.

Education

Physical therapy degree (B.S., M.S., or D.P.T)

State licensure as a Physical Therapist

CPR/Basic Life Support (BLS) certification
