Job summary

Fairview East Bank University Hospital in Minneapolis is seeking a casual physical therapist to support acute care patient needs. You will collaborate with patients and families to provide safe, effective care, including evaluation, diagnosis, planning, intervention, and outcomes tracking.

The role involves per diem scheduling with weekend coverage and opportunities to pick up additional shifts; licensure in Minnesota and BLS are required.

Qualifications

• Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
• Basic Life Support (American Heart Association or Red Cross)

Responsibilities

• Partners with patient and family during Evaluation/Re-evaluation
• Gathers pertinent data
• Performs examination
• Determines treatment diagnosis
• Develops an individualized plan of care with patient and family
• Coordinates care and communicates with interdisciplinary team for all aspects of patient care
• Establishes appropriate discharge plan
• Implements and modifies plan of care based on reassessment and patient response
