Job summary

Fairview Health Services seeks a core physical therapist to support acute care patient needs at Fairview Southdale Hospital in Edina. You will work with a diverse patient population including ortho, neuro, general medical and surgical, cardiac and oncology, delivering safe and effective care within your scope.

The position is a 0.775 FTE (62 hrs per pay period) with a schedule of Thursdays and Fridays and two full weekends per month (9-hour shifts) and one holiday per year.

Qualifications

• Minnesota Physical Therapy License required (or temporary license)
• Basic Life Support (American Heart Association or Red Cross) certification required
• Doctorate of Physical Therapy preferred; 1+ year experience; 1+ year acute care experience

Responsibilities

• Gathers pertinent data and performs Evaluation/Re-evaluation
• Develops an individualized plan of care in collaboration with patient/family
• Implements and adjusts plan of care based on patient response and reassessment
• Coordinates care and communicates with interdisciplinary team for all aspects of patient care
• Establishes discharge plan and patient/family education
