Banner Estrella Medical Center in Phoenix, AZ is seeking a part-time Occupational Therapist to join our acute care team. Qualifications

Must have completed an OT program approved by the governing board.
Must possess a current Occupational Therapy license in the practicing state.
Current BLS certification required.
Must be able to use computer software and electronic patient records.
Arizona Fingerprint Clearance Card required at hire and maintained throughout employment.

Responsibilities

Evaluate patient functional abilities and determine need for intervention.
Develop and implement an individualized plan of care with measurable goals.
Document assessments and treatment plans according to departmental standards.
Communicate results and progress with physicians, nurses, patients and families.
Provide education to patients, families and the care team.
