Medical Center Health System is seeking a Social Worker to provide comprehensive psychosocial assessment, intervention, resource coordination and discharge planning for patients and families. You will collaborate with case managers, physicians, nurses, therapists, community agencies, and other team members to overcome barriers to safe discharge and ensure continuity of care.

Required is a Master’s degree in social work with Texas licensure preferred; at least one year of acute care experience is
