Medical Center Hospital is seeking a Social Worker to provide comprehensive psychosocial assessment, intervention, resource coordination and discharge planning for patients and families. You will collaborate with case managers, physicians, nurses, therapists and community agencies to remove barriers to safe discharge and ensure continuity of care.

• Master’s degree in social work is required.
• Texas license preferred.
• Minimum one year experience in acute care preferred.

• Ability to tolerate frequent interruptions and heavy workload.

The Social Worker Is responsible for providing comprehensive psychosocialassessment, intervention, resource coordination and discharge planning servicesto patients and families. Collaborate with case managers, physicians, nurses,therapists, community agency members, and other interdisciplinary teammembers to address barriers to safe and appropriate discharge and promotecontinuity of care.

Education and Certification

• 1. Master’s degree in social work required


• 2. Texas license preferred.

Training and Experience

• 1. A minimum of one-year experience in acute care setting is preferred.


• 2. Excellent verbal and written skills, well-rounded critical thinking skills,self-motivated, flexible, and goal-oriented.


• 3. Ability to tolerate frequent interruptions and demanding workload.

Job Knowledge

• 1. Demonstrates knowledge of the principles of growth and developmentover the life span and possesses ability to assess data reflective of thepatient’s status and interprets the appropriate information needed toidentify each patient’s requirements relative to his/her age specificneeds.
