Job summary

Qualifications

• Graduate of an accredited Master of Social Work program (MSW).
• Current license as a Master Social Worker (LMSW) in Texas; ACM certification preferred.

Responsibilities

• Assesses patient’s and family’s psychosocial risk factors and coordinates care.
• Develops and carries out treatment plans using clinical social work diagnoses and interventions.
• Intervenes with emotional, social and financial consequences of illness; mobilizes community resources for identified needs.
• Provides counseling related to post-acute options and end-of-life issues.
• Educates patient/family and physician regarding post-acute options and discharge planning status.
