Medical Center Health System is seeking a Social Worker to provide comprehensive psychosocial assessment, intervention, resource coordination and discharge planning for patients and families. You will collaborate with case managers, physicians, nurses, therapists, community agencies, and other team members to overcome barriers to safe discharge and ensure continuity of care.

Required is a Master’s degree in social work with Texas licensure preferred; at least one year of acute care experience is

Position Summary

The Social Worker Is responsible for providing comprehensive psychosocial assessment, intervention, resource coordination and discharge planning services to patients and families. Collaborate with case managers, physicians, nurses, therapists, community agency members, and other interdisciplinary team members to address barriers to safe and appropriate discharge and promote continuity of care.

Position Summary

Collaborate with case managers, physicians, nurses, therapists, community agency members, and other interdisciplinary team members to address barriers to safe and appropriate discharge and promote continuity of care.

Education And Certification

• Master’s degree in social work required
• Texas license preferred.

Training And Experience

• A minimum of one-year experience in acute care setting is preferred.
• Excellent verbal and written skills, well-rounded critical thinking skills, self-motivated, flexible, and goal-oriented.
• Ability to tolerate frequent interruptions and demanding workload.

Job Knowledge

• Demonstrates knowledge of the principles of growth and development over the life span and possesses ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs.

Unusual Physical Demands and Working Conditions:

• Must be able to move about hospital without major constraints.
• Universal precautions are always maintained.
• Must be able to work long hours on occasion.
