Qualifications

• Active LMSW credential or licensure.
• Minimum 2 years' experience in acute care or medical/surgical social work.
• Experience in hospital or healthcare settings.
• Strong assessment, case management, and crisis intervention skills.
• Excellent communication and interdisciplinary collaboration abilities.
• Ability to work independently and adapt to a fast-paced environment.

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans for patients in acute care/medical/surgical environments.
• Coordinate with healthcare providers, patients, and families to facilitate discharge planning and access to community resources.
• Provide crisis intervention, counseling, and support to patients and families facing complex medical and social challenges.
• Document assessments, interventions, and patient progress accurately in compliance with regulatory standards.
• Collaborate with interdisciplinary teams to promote holistic patient care and optimize outcomes.
• Participate in coverage for weekends and holiday shifts as required.
