A minimum of one year in acute care and current Washington state licensure with BLS is required. The successful candidate will float between Colby and Pacific campuses, perform assessments, coordinate transitions, and ensure appropriate

• Master of Social Work required and WA state LCSW license.
• Minimum one year of acute care social work experience.
• Proficiency with Epic EMR and BLS certification.
• Strong discharge planning and care coordination skills.
• Ability to collaborate with interdisciplinary teams and adapt to hospital policies.

Responsabilités

• Perform acute care social work assessments and interventions.
• Coordinate discharge planning and facilitate care transitions.
• Collaborate with medical, nursing, and ancillary staff.
• Document social work activities and patient progress in Epic.
