Job summary

• Upon hire: Washington Clinical Independent Social Worker License
• 2 years SW experience in any healthcare setting
• 1 year experience in acute care setting or TIPs/Case Management Orientation Program

Responsibilities

• Assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy.
• Ensure continuity of care for vulnerable patients by addressing barriers to accessing resources and care.
