Job summary

Mercy Health at The Jewish Hospital in Cincinnati seeks a Clinical Pharmacy Specialist to deliver patient-centered pharmaceutical care, coordinate clinical programs, and ensure safe, cost-effective therapy with a strong emphasis on collaboration across the health care team.

The role requires a PharmD (ACPE-accredited) and Ohio licensure, with at least 2 years in acute care and leadership experience preferred; EPIC EHR experience is a plus, supporting a culture of excellence and patient safety.

Qualifications

• Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required).
• Advanced pharmacy degree – Doctor of Pharmacy, Masters, Residency (preferred).

Responsibilities

• Conduct initial and ongoing patient assessment with interdisciplinary care team.
• Administer medications safely per state practice and policies.
• Serve as point of contact for patient care coordination.
• Participate in quality improvement and ensure regulatory compliance.
