Job summary

Requirements include PharmD, NY license, ACPE-accredited school, and PGY1/PGY2 residency; board certification preferred.

Qualifications

PharmD degree required with PGY1 in acute/ambulatory care; PGY2 preferred; board cert within 1 year of hire.
New York State license to practice Pharmacy and current registration.
ACPE-accredited school recognized by BPS/ASHP; residency experience encouraged.
Residency completion verification via ASHP database or site certificate required.
Ongoing maintenance of board certification in clinical inpatient care is expected.

Responsibilities

Develop and maintain a comprehensive Clinical Pharmacy Services Program across ED, ICU, ID, psychiatry, ambulatory care, oncology, and internal medicine.
Precept pharmacy residents and provide educational programs for staff and patients.
Regulatory readiness and compliance with statutory regulations.
Collaborate with system and site leadership to ensure readiness and optimal patient care.
Provide educational programs for nurses, clinicians, residents, and families.
