The incumbent will participate in drug monitoring programs, educate staff and patients, and respond to emergencies as needed, all while maintaining accurate records and complying with policies and regulations.

Qualifikationen

• Current license to practice pharmacy in the State of Illinois.
• Bachelor's Degree in Pharmacy or Doctor of Pharmacy Degree.
• Residency training or hospital pharmacy experience preferred.

• Interprets medication orders to ensure safety and proper use of medications and alerts to interactions or allergies.
• Coordinates solving of medication-related problems between pharmacy, nursing, and prescribers.
• Monitors medication therapy for drug interactions and adjusts as indicated by patient condition and lab results.
• Evaluates and documents that selected medications achieve the desired therapeutic outcome.
• Communicates potential problems and solutions to prescribers and educates patients as needed.
• Responds to emergency situations to assist with medication management.
