Job summary

Qualifications

• Must have acute care and med-surg experience.
• Experience with charting systems, specifically Epic.
• Ability to work some weekends and holidays.

Responsibilities

• Conduct initial evaluations and develop care plans to meet patient and caregiver needs.
• Facilitate discharge planning and coordinate services to home care and equipment.
• Collaborate with interdisciplinary teams to support patient care.
• Document plan of care and patient teaching with strong accuracy.
• Travel between facilities as needed and cover some major holidays.
