Job summary

Qualifications

• WA license required to practice social work in the state of Washington.
• Acute care experience is mandatory for this role.
• Travel experience is required and previous travel assignments are preferred.
• Epic is the charting system used; familiarity is required.
• AIDA EMR experience is preferred.
• BLS certification is required.

Responsibilities

• Discharge planning and care coordination across hospital units.
• Coordinate with internal/external teams to ensure timely patient transfers.
• Document care plans and patient progress in the charting system.
• Provide case management support in various hospital settings.
