Qualifications

Responsibilities

• Conduct biopsychosocial assessments.
• Coordinate care for patients transitioning to home care and other facilities after discharge.
• Support discharges from ED/ICU with appropriate care plans.
• Document care delivery accurately and maintain clear communication.
• Be flexible to float to different areas of the organization.
