Description:Candidates must hold a Master of Social Work degree.Acute care experience and discharge planning/care coordination experience are required.Must be able to work every other weekend.The role involves working with a varied patient load, with the average case load being 18-24 patients, and may increase to 30 patients on weekends.Clinical duties will include conducting biopsychosocial assessments and coordinated care for patients transitioning to home care and other facilities after discharge.Experience in emergency department and ICU settings is preferred.Candidates must be flexible and willing to float to different areas of the organization.A valid Washington state license is required prior to start.The position mandates strong interpersonal skills and the ability to document care delivery accurately.Commitment to working three out of five major holidays is expected.Participants will need to respond to specific inquiries regarding past experience with any facilities.References covering the last year of experience are necessary during the compliance process.Travelers may expect to float between facilities as required.
