Job summary

Kindred Hospital San Antonio Central seeks a qualified Care Coordinator to manage patient care across the continuum and collaborate with the interdisciplinary team. You will facilitate effective care transitions, advocate for patients, and support optimal resource use.

The role requires knowledge of payor practices and regulatory standards, with strong communication and MS Office skills; experience in case management or discharge planning is a plus.

Qualifications

• Graduate of an accredited program: RN or LPN/LVN.
• Master of Social Work or Bachelor of Social Work with licensure per state rules.
• Licenses/certifications as required by state regulations.

Responsibilities

• Coordinate care with ICT team and physicians to manage patient flow.
• Assist with discharge planning and transition of care.
• Apply knowledge of reimbursement and payor requirements to patient care.
• Advocate for patients and families while ensuring quality care.
• Collaborate with interdisciplinary staff to implement care plans.
