Job summary

SummitStone Health Partners in Fort Collins, CO seeks a Care Coordinator – Acute Care to support clients transitioning from acute care settings. You will educate, outreach, and connect clients to MAT and community supports to help meet recovery goals.

The role requires at least 1 year of experience in care coordination or case management within behavioral health or substance use, plus a high school diploma. Occasional travel to client homes and partner sites may be required.

Qualifications

• High school diploma or GED required.
• At least 1 year of care coordination or case management experience in mental health, substance use, or related field.
• Experience with mental illness and substance use disorders; ability to navigate care systems.

Responsibilities

• Provide care coordination services in collaboration with clients.
• Engage with emergency departments, primary care, and behavioral health teams to support continuity of care.
• Outreach clients on referral or cold outreach basis.
• Participate in Certified Addiction Specialist coursework.
• Collaborate with SummitStone staff and CO-SLAW network.
• Assist clients in navigating healthcare and community systems for discharge and follow-up.
• Assess client needs with acute care teams to determine post-discharge services.
• Coordinate with internal teams and community agencies to identify resources.
