Job summary

SummitStone Health Partners in Fort Collins, CO, seeks an Advanced Practice Provider for day shifts to join our new acute care behavioral health facility. The role covers assessment and treatment of substance use and mental health disorders in inpatient and outpatient settings.

The position requires a Master’s degree in APN/PA, 4+ years of related experience, and a Colorado license with DEA; buprenorphine-waiver is expected within 30 days of hire.

Qualifications

• Must-have background in behavioral health and substance use treatment.
• Experience with medication-assisted treatment and harm-reduction principles.
• Licensed in Colorado as APN/PA with DEA and buprenorphine-waiver readiness.
• Strong communication and team collaboration skills.

Responsibilities

• Serve as medical team representative within a multidisciplinary treatment team.
• Evaluate patients for substance use, mental health, and medical disorders; determine treatment.
• Provide direct patient care in inpatient and outpatient settings.
• Diagnose, treat, and prescribe medications for acute and chronic illnesses with behavioral health focus.
• Assess health through interviews, exams, and med history reviews.
• Manage acute and chronic behavioral health and substance use conditions.
• Initiate emergency measures when needed and stabilize patients.
• Document care and maintain patient records.
• Educate patients about regimens, growth, wellness, and reproductive health.
