Job summary

Monarch Healthcare in North Carolina is seeking a qualified Assertive Community Treatment Team Substance Abuse Professional. This role requires a Master’s in Human Services, LCAS credentials, and experience coordinating services for individuals with mental health and substance use challenges.

You will work within a multidisciplinary team to create and implement person-centered plans, coordinate community resources, and support independent living skills while traveling as needed.

Qualifications

• Experience providing employment services.
• Experience working with adults with a mental health diagnosis or substance use disorder.
• 1–4 years of related experience (dependent on education).

Responsibilities

• Provide service coordination for individuals with mental health and substance use issues and develop the overall service plan.
• Assist in the development of natural support networks and provide counseling and link to services.
• Write, implement, evaluate, and revise Person Centered Plans and treatment goals.
• Work as a multidisciplinary team to develop and implement treatment goals.
• Facilitate training of independent living skills and assess daily needs.
• Coordinate services with the program staff, facilities, and community resources.
• Review progress and provide ongoing consultation with team and agencies.
• Act as liaison with after-hours staff and share participant information.
• Meet regularly to review caseload and assignments.
• Driving and travel may be required.
