Job summary

Lakewood Nursing & Rehabilitation Center in Illinois is seeking a dynamic Activity Aide to enhance daily living for residents. You will facilitate group and individual activity programs based on residents’ needs and care plans, and help manage the activity calendar.

Experience in social or recreational programs in healthcare is preferred; you will document admissions/discharges, promote resident participation, and support Resident Council activities.

Qualifications

• 1-2 years of experience in a social or recreational program in a healthcare setting (preferred).

Responsibilities

• Facilitate ongoing group and individual activity programs based on residents’ needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events.
• Follow and maintain established policies and procedures.
• Encourage resident participation in Resident Council and schedule monthly meetings.
