Job summary

Lemont Nursing and Rehabilitation Center is seeking a dedicated Activity Aide to bring daily living to new levels for residents. You will lead engaging activities, interact with residents and staff, and help create a vibrant, social environment that supports recovery and well-being.

With 1-2 years of healthcare activity experience, you will document care plans, assist in planning monthly calendars, and uphold facility policies while encouraging resident participation in council activities.

Qualifications

• 1 - 2 years of experience in a social or recreational program in a healthcare setting (preferred).

Responsibilities

• Facilitate group and individual activity programs based on residents' needs and care plans.
• Complete required resident admission, discharge, and transfer records per policy.
• Create activity calendars and coordinate events.
• Follow established policies and procedures.
• Encourage Resident Council participation and monthly meetings scheduling.

Activity Aide Requirements

• 1 - 2 years of experience in a social or recreational program in a healthcare setting (preferred).

As an Activity Aide, you will

• Facilitate ongoing group and individual activity programs based on residents’ needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events.
